Nursing Home Notary — Dallas–Fort Worth
Request first. Book after confirmation. Independent providers confirm availability and their own rates. No payment is collected on this form.
Coordinate the visit with the facility
Before requesting a nursing-home notary, ask the facility which entrance visitors should use, whether an appointment is required and what visiting hours apply. Name an on-site contact who can help with access. Include the facility name, city and ZIP in the inquiry; share room-level directions directly with the assigned provider when needed.
Choose a time that works for the resident, rather than only for the family member arranging the appointment. Allow room for therapy, meals and other care routines. If the schedule changes, tell the provider before travel begins.
Prepare the document and people
- Have the complete document prepared before booking; the referral service does not draft it.
- Confirm who must sign and whether anyone else must attend.
- Ask the provider to review identification arrangements in advance.
- Check witness instructions with the document preparer and ask the facility whether staff may participate.
- Keep the resident involved in scheduling and confirm they want the visit.
For medical powers of attorney and other prepared documents
If a family member needs a medical power of attorney notarized at a nursing home, describe that document type in the request. Do not include a diagnosis or upload medical records. A mobile notary appointment is for the notarial act; it does not create the document or replace legal guidance about the signer’s circumstances.
Read our medical power-of-attorney appointment guide. For an inpatient setting, see hospital notary visits.
Avoid a preventable second trip
Tell the provider if the document is still being prepared, if identification is missing or if the visit time is uncertain. These issues may need to be resolved before an appointment can proceed. A family member’s request alone does not guarantee that the notarization can be completed.
Ask for the complete itemized quote, including travel and any waiting policy. Confirm what happens if access is refused or the appointment cannot proceed. We do not claim affiliation with any facility or guarantee admission.
